This page tells you the terms on which you may use our website, whether as a registered user or guest. Please read carefully before use.
By using the site, you accept the terms and agree to obey them. If you don’t accept them, please don’t use the site. Please read these Terms and Conditions carefully.
These Terms and Conditions are subject to change. Updated terms will only apply to programmes or courses made after the changes, if you are part way through a programme or course your original terms will remain unchanged.
It is essential that all information provided by you when completing your medical history and PARQ questionnaire is fully accurate as non-disclosure of information may invalidate your agreement with us.
Consultation Fees

Team Clinic Services offers a client-centred remote clinic programme that prides itself on offering top quality advice and support. Programmes and courses are competitively priced based on our team's extensive experience and expertise.
We aim to provide our clients with in-depth information about our methods and courses to help them make informed decisions. Team Clinic Services works closely with our consultants and clinicians on a regular basis and can help with any initial questions that you may have prior to your appointment.
Payment is required for all courses, appointments and associated consultations. Your appointment will be extensive in nature and delivered by a highly and appropriately qualified clinician.
The cost of your consultation or course will have been quoted by a Team Clinic Services team member, if you require further consultations or support the relevant fees associated with these will be provided prior to the appointment. Please ensure you are aware of the fees involved in advance of consultations and courses and if unsure kindly ask a Team Clinic Services team member.
Payment

You are required to pay the total cost of your consultation on request and in advance of consultations taking place.
Payment can be made via Credit or Debit card. Team Clinic Services does not offer any finance options.
Payment of Invoices
​
Payments are to be paid on receipt of the invoice. Late payment or failure to pay the invoice within the allotted time prior to the consultation taking place will result in the cancellation of your appointment. Any further appointments would require immediate payment at the point of booking using a credit or debit card.
Cancellations and Failure to Attend your Appointment
​
Cancellations can be made by phone or email at least 48 hours prior to your scheduled appointment.
We would be very grateful if you cancel as soon as possible to avoid any confusion and to help us maintain our level of service.
Failure to cancel an appointment within 48 hours of the scheduled time will result in you being liable for the full cost of the appointment and no refund will be provided. For a first time cancellation, we will allow you to reschedule your appointment as long as it is attended within 14 days of the cancellation without charge.
Remote Consultations

Team Clinic Services offers remote consultations/appointments. Appointments may take place via telephone or online video calls. You are advised to ensure that you are using a secure internet connection. Team Clinic Services can take no responsibility for the quality of the call if your wifi connection is poor. Appointments are to be held in a quiet area with adequate privacy. The appointment must not be recorded or retained by you unless this has been consented to by yourself and Team Clinic Services. Team Clinic Services reserves the right not to offer remote consultations if it is deemed clinically inappropriate. Payment is due upon booking the remote appointment.

What To Expect From Your Therapist/Coach:
​
a. A full consultation form should be completed prior to treatments and should include brief medical history including past/ present injuries and exercise history, signed and dated by you.
b. Your therapist must maintain a professional relationship with you conducting themselves in a courteous and dignified manner at all times, be open and honest and act with integrity.
c. If your therapist , coach, trainer is running late for your scheduled appointment, you will be informed of the estimated time of arrival be phone call or text message.
d. If your therapist, coach, trainer needs to cancel your appointment every effort will be made to find an alternative appointment.
e. Your Therapist, coach, trainer should provide an ethical assessment and make no false claims.
f. Your therapist , coach,trainer has a moral and legal duty to protect themselves from harm and take reasonable measures to protect their own health, well-being and physical safety whilst working.
g. Your therapist, coach , trainer has a legal duty to protect you from harm and your safety is paramount and all efforts must be made to ensure you come to no harm whilst being treated.
h. Your Therapist, coach, trainer is expected to practice high standards of hygiene and take all efforts to ensure that standard health practices are maintained.
i. All records, information and conversations will be entirely confidential and not disclosed to any third party without your consent, except in extreme circumstances and where it is required by law to do so.
j. Team Clinic Services and or trainers, therapists , coaches reserve the right to deny services to anyone who they find unable to receive treatments or training due to health contraindications that come to light during consultations.
What Team Clinic Services Expects In Return

a.You are authorised by your Doctor or specialist to receive treatments and or participate in any training programme, course and consultations
b. You are required to complete the proper documentation prior to participation within Team Clinic Services health and well-being programmes, consultations.
d. You agree to treat your therapist/trainer/coach with respect and that any aggressive or abusive behavior will not be tolerated and may result in legal action.
Child Policy (Under 18's)

In line with the Care Quality Commission regulations, we are unable to treat persons under the age of 18 years.

WAIVER OF LIABILITY (PLEASE READ CAREFULLY)
It is advised to have a free discovery call prior to purchasing your course or consultations.If you choose to take part in the First To Fit 12 week course without a free discovery call you do so at your own risk
a. This agreement releases the trainer/therapist/coach from all liability relating to sickness or injuries that may occur during partaking in the any consultations and courses, you agree to hold Team Clinic Services or its trainers, therapists, coaches entirely free from any liability, including financial responsibility for sickness or injuries incurred, regardless of whether sickness or injuries are caused by negligence.
b. You hereby affirm that you are in good physical condition and do not suffer from any disability that would prevent or limit participation in this program.
c. In consideration of your participation in the consultations and courses, you and your stakeholders hereby release Julia Pizzichemi and Team Clinic Services stakeholders from any and all claims, demands or causes of action arising from your participation in the consultations and courses or from any use of the conditioning and exercise equipment and facilities...
d. You promise that you are participating voluntarily, and that all risks have been made clear to me.
e. Additionally, you do not have any conditions that will increase the likelihood of experiencing sickness or injuries while engaging in any activity.
f. You understand you forfeit all right to bring a suit against Julia Pizzichemi and Team Clinic Services for any reason. In return, you will also make every effort to obey safety precautions as explained to you verbally. You will ask for clarification when needed.
